Strange PDF header? problem

I'm not ruling out that the original PDF might be bad because it was 
created by a competing paper that is NOTORIOUS for releasing bad work. 
The thing I don't understand is why does it work 99 times, then the 
100th time messes up and every time following the 100th gives the same 
result. Something has changed, but it is not the original PDF. (Maybe 
some circumstance that "brought out" the bad post scripting on the 100th 
try?)
